Our primary goal is to meet Carers within the hospital settings and offer assistance to prevent carer breakdown. We also support the safe discharge process for both the carer and patient. Working closely with health and social care colleagues, we identify Carers early in their caring role to ensure they can access the appropriate support they need.

When Admitted into Hospital

During a hospital admission, whether it's planned or an emergency, we understand it can be a challenging time for everyone involved. If you have a friend or relative admitted, or if you are a caregiver and find yourself admitted to the hospital, our dedicated on-site team is here to offer you support.

We are committed to helping you navigate the hospital systems, ensuring you feel fully included in the discharge planning process. Our aim is to provide you with access to all the support available, tailored to meet your individual needs. Additionally, we now offer interim beds for patients who have been relocated before their final discharge destination, ensuring a seamless transition. Rest assured, our hospital team is here to assist you every step of the way.

Hospital Discharge Planning Support

Our teams are here to guide you through the discharge process, ensuring you have a clear understanding of the procedures. We make sure that your wishes and thoughts are effectively communicated to the hospital professionals, and we actively engage in discussions concerning the patient and their family’s concerns.

To provide the best support possible, we collaborate with various partner charities across West Sussex. Together, we strive to ensure that each carer receives the appropriate assistance upon discharge, tailored to their individual needs. Our partner charities include:

Support at Home after Hospital Service

The Support at Home after Hospital service is delivered for Carers in West Sussex by Carers Support, working with Age UK West Sussex Brighton and Hove. Our aims is to support Carers themselves or the person, they give care to who has recently been discharged from hospital. Our team will carry out phone or in home assessments to ensure we are able to put the right support in place to help the person you care for, regain independence, access appropriate services and equipment to meet their needs.

Carers Support Hospital Service operates within 13 Acute and Community hospitals serving West Sussex residence, including East Surrey, Princess Royal, Royal Sussex County, St Richards and Worthing General Hospitals and the county’s many community hospitals.

Worthing Hospital

We have five support workers attached to this hospital who will be happy to meet you and support with your time in Worthing Hospital. St Richard’s Hospital – Chichester

We have three support workers attached to this hospital who will be happy to meet you and support with your time in St Richard’s Hospital. Princess Royal Hospital – Haywards Heath

We have two support workers attached to this hospital who will be happy to meet you and support with your time in Princess Royal Hospital. East Surrey Hospital – Redhill

East Surrey hospital has a Carers Passport to allow carer of patients to access support and care when visiting the hospital. When the person you care for is admitted onto a ward, identify yourself to a ward sister as the main carer and request a Carers Passport.
